Vilitra (a vardenafil?based PDE5 inhibitor) interacts with nitrate medications (like nitroglycerin and other nitrates used for chest pain/angina) Strong Interaction — Do Not Take Together Vilitra works by relaxing blood vessels to increase blood flow to the penis. This also lowers blood pressure to some degree. Nitrates (such as nitroglycerin, isosorbide mono?/dinitrate, amyl nitrite) also dilate blood vessels to relieve chest pain. Combining Vilitra with nitrates can cause a dangerous Vilitra rapid drop in blood pressure (hypotension), which may lead to dizziness, fainting, heart attack, or even life?threatening cardiovascular collapse. Because of this, the combination is medically contraindicated — meaning it should not be used. Why This Happens Both drug classes increase levels of cyclic GMP (cGMP) in blood vessel walls: Nitrates boost nitric oxide ? more cGMP ? vasodilation. PDE5 inhibitors (like vardenafil in Vilitra) block the breakdown of cGMP ? stronger/more prolonged vasodilation. Together, the effect is too much vasodilation ? severe drop in blood pressure. ? Timing Between Drugs If someone has inadvertently taken a PDE5 inhibitor: For short?acting PDE5 inhibitors (like vardenafil), doctors typically recommend waiting at least 24?hours before using nitrates if absolutely medically necessary. Even then, this should only be done under strict medical supervision. Bottom Line Vilitra and nitrates should never be taken at the same time. If you are prescribed nitrates for chest pain or heart problems, tell your doctor before using Vilitra or any ED medication. Combining them can cause dangerously low blood pressure and serious cardiovascular issues.
